Is a sports physical for your child on your to-do list? Not to worry: A sports physical doesn’t take much longer than a regular check-up. Your child’s doctor or nurse will provide a questionnaire asking about health and family history, then review the results with you and do a few quick exams to ensure they’re ready for gameday. You can even save time by combining a sports physical with your annual well-child visit. M Health Fairview has over 40 primary care clinics. Find out how you can schedule a physical at our nearest neighborhood location. Learn more.
Yes. Well-child checkups are routine preventive appointments, and they’re important for your child’s health, development, and well-being. The appointments are an opportunity for your child’s doctor to check basic health indicators, potentially catching medical concerns early on or stopping them before they start. Your child’s provider can make helpful recommendations about sleep, nutrition, and activity for your child, and it’s also a great opportunity for you to ask questions. Standard immunizations, on a timeline recommended by the state health department, are also part of well-child care. Learn more.
Yes. M Health Fairview is the largest provider of mental health and addiction services in the Upper Midwest. We offer a complete range of mental health and addiction programs and services for children, adolescents, and young adults. And because we prioritize easy access to mental health and addiction services, some of our team members are embedded in M Health Fairview primary care clinics, to identify and address needs at their earliest and most manageable stages. Pediatricians are doctors who care for children exclusively, from birth through age 18. Family medicine specialists are doctors who care for people of all ages, from newborn babies through older adults. M Health Fairview offers access to both types of doctors to better meet your family’s unique needs. Our range of primary care providers also includes nurse practitioners or physician assistants who specialize in pediatrics or family medicine.

When you have “proxy access” to MyChart, you’re able to view another person’s MyChart information and take actions in their account (like view lab test results, read provider notes, and make appointments).
Parents or legal guardians are given full proxy access to MyChart records for children age 0-11. But under Minnesota law, minors ages 12 and older have the right to control the release of information related to pregnancy, STD, physical or sexual abuse, and substance abuse diagnosis or treatment.
If your child agrees to grant you proxy access to your child’s medical record, you can continue to view your child’s full medical records. Otherwise, you will only receive partial access. Here is how the process works:
- Before your child turns 12, talk with your child’s primary care provider about getting proxy access to your child’s MyChart account.
- The process of granting proxy access has to be done as part of an in-person visit.
- Your child’s doctor will talk with your child while you’re out of the room about what proxy access means and whether your child wants to grant you full or partial proxy access.
- If your child agrees to grant full access, your child and your provider will sign a consent form. Then, you are granted full proxy access to your child’s medical record.
- If your child does not agree, you will receive only partial proxy access.
- If you don’t complete this process before your child turns 12, it’s OK. You can still follow the process above to get proxy access. Be aware, however, that until the consent form is signed, you will not have full access to your child’s medical record.
