The Fairview System Credentialing Office (FSCO) provides a wide range of benefits to its contracted facilities and networks:
- It streamlines the credentialing process so practitioners submit one credentialing application and privilege form(s) when applying to one or multiple M Health Fairview and/or contracted facilities/networks
- It eliminates duplication of multiple verifications
- It standardizes privileging criteria and forms
- It provides ongoing license monitoring, malpractice insurance, DEA certification, and board certification for all applicable practitioners.
- It maintains compliance with Joint Commission (JC), Centers for Medicare/Medicaid Services (CMS), National Committee for Quality Assurance (NCQA) credentialing standards

About the Fairview System Credentialing Office (FSCO)
The Fairview System Credentialing Office's (FSCO) mission is to conduct thorough and accurate verification and review of provider credentialing and privileging information. This work supports the medical staff at Fairview Health Services and contracted facilities and networks by thoroughly assessing practitioners' ability to provide quality patient care and minimizing risk to the organization.

The initial credentialing and privileging process has many steps and detailed requirements. In summary, this process includes:
- An application request form is submitted. Login instructions are emailed to the applicant to access the ‘Practitioner Hub’ where electronic application forms are completed.
- When all required forms are completed, a Credentialing Coordinator will be assigned to work with the applicant to assure completion of all required documents, verifications, and privilege criteria.
- Requests for information will be sent to the applicant by email. Various other departments may also be in contact with the applicant, such as Recruitment, Human Resources, Provider Enrollment, Department Contacts, and Employee Occupational Health Services. Requests from other departments may be similar to the information requested by the FSCO as needed for their specific purposes.
- After all verifications and required documents have been received, the application is sent to each facility where credentialing and/or privileges is being requested. Applicants are notified when their application is being sent for the review and approval process.
- The application is reviewed by Medical Staff Services at the facility(ies) requested and sent to the designated Credentials Reviewer for review and recommendation, then to the Credentials Committee for review and recommendation, then to the Medical Executive Committee (if applicable), and the facility(ies) Board for approval (if applicable).
- When approval has been granted, Medical Staff Services at each facility sends a Board Letter to notify applicants of the details of their approval and effective date at that facility.
- After approval has been granted, practitioners may begin caring for patients within the scope of their approval.
- Practitioners are expected to read and be familiar with the Bylaws and/or governing documents from each facility. This information will be provided at the time of approval and is available on the Fairview Intranet.
Practitioners are reappointed at least every 36 months. This credentialing reappointment schedule is aligned to be approved the month of the practitioner's birth.
Reappointment Application Process Steps:
- The FSCO sends an email to practitioners approximately four and a half months before their current appointment expiration date with login instructions to access the Practitioner Hub and complete the electronic reappointment application forms.
- Practitioners review and confirm that the demographic information in the packet is correct and complete new disclosure questions, authorization and release, attestations for Medicare/Medicaid, and continuing education information.
- Practitioners review and make changes to privileges (if applicable) and submit any required documentation for the privileges.
- The medical staff services teams at the facility(ies) requested review the application and send it to the designated credentials reviewer for review and recommendation.
- The application goes to the credentials committee for review and recommendation, then to the medical executive committee (if applicable) and the facility’s board for approval (if applicable).

The FSCO monitors the following items as applicable to each practitioner to assure they are kept current and in good standing:
- State license
- Malpractice insurance
- DEA certification
- Board certification
Governing documents require practitioners to maintain a current license, insurance, and certification at all times. Failure to renew state license or malpractice insurance prior to its expiration will result in suspension of medical staff credentialing, privileges and/or membership at M Health Fairview until renewal is appropriately verified.
If a practitioner’s license or insurance expires, the practitioner will be made temporarily inactive and will not be able to see patients at any M Health Fairview facility. If these credentials or certificates are not renewed or cannot be verified within 90 days of expiration, the practitioner’s temporary inactive status will become a permanent inactive status. Future credentialing requests will require the practitioner to submit an initial application again.
The FSCO has delegated credentialing agreements with the health plans listed below, allowing the health plans to accept the Fairview’s credentialing process without the need for their own separate credentialing. Fairview’s approval date will be used as the enrollment date for the noted health plans for the applicable practitioners.
The FSCO is accredited by the National Committee of Quality Assurance (NCQA) in Credentialing and Recredentialing. This certification is required to maintain the delegated credentialing agreements.
The delegated credentialing process has several benefits. It streamlines processes and reduces costs for both practitioners and health plans. It also allows applicable practitioners to start providing services as soon as their credentialing is approved at Fairview without needing to wait for separate approvals from the delegated health plans.

Important information for all practitioners
All credentialed practitioners are expected to read and be aware of the information contained in the following documents:
Proctoring
Proctoring privileges may be requested if a practitioner does not meet the criteria indicated on the privilege form, such as number of cases performed or required training. All required training must be completed before a proctoring request is made. Proctoring is an evaluation of a practitioner’s clinical competency to perform the procedure after completion of required training and must be approved in advance of performing the procedure with proctoring. Required learning
All practitioners credentialed at a Fairview facility are required to complete the designated medical staff education. Following approval of your credentialing, you will receive an email containing a link to Fairview’s Learning Management System (LMS), where you will complete the required training.
