Your child's medical records are protected health information, and we take our responsibility to protect that information, and your child's privacy serious. We diligently abide by all state and federal regulations, including the Health Insurance Portability and Accountability Act, or HIPAA.

Medical Record Restrictions for Minors 12+
Parents or legal guardians are given full proxy access to MyChart records for children age 0-11. But under Minnesota law, minors ages 12 and older have the right to control the release of information related to pregnancy, STD, physical or sexual abuse, and substance abuse diagnosis or treatment.
If your child is between the ages of 12 and 17, you will be granted partial, limited proxy access to your child’s medical record upon request unless your child chooses to allow you full access.
If you need a copy of your 12+ child’s medical record, contact our health information management team for assistance at 952-924-5165 or releaseofinformation@fairview.org from 8 a.m. to 4:30 p.m., Monday through Friday. What is “proxy access” to MyChart?
When you have “proxy access” to MyChart, you’re able to view another person’s MyChart medical record information and take actions in their account like view lab test results, read provider notes, and make appointments. You are acting on that person’s behalf, which is called acting as that person’s “proxy.” You can arrange to have proxy access for your children or other members of your family. That means you can manage your whole family’s health needs in one place.
How do I get proxy access to MyChart for my older child?
According to Minnesota law, children ages 12 to 17 have the right to decide who has access to their medical records related to pregnancy, STD, physical and sexual abuse, and substance abuse diagnosis and treatment. Parents or guardians will have partial, or limited, proxy access excluding these areas unless your child specifically agrees to grant you full proxy access. Here is how the proxy access process works for children 12-17:
- Before your child turns 12, talk with your child’s primary care provider about getting proxy access to your child’s MyChart account.
- The process of granting proxy access has to be done as part of an in-person visit.
- Your child’s doctor will talk with your child while you’re out of the room about what proxy access means and whether your child wants to grant you full or partial proxy access.
- If your child agrees to grant full access, your child and your provider will sign a consent form. Then, you are granted full proxy access to your child’s medical record. If your child does not agree, you will receive only partial or limited proxy access.
- If you don’t complete this process before your child turns 12, it’s OK. You can still follow the process above to get proxy access. Be aware, however, that until the consent form is signed, you will not have full access to your child’s medical record.
Requesting Medical Records through MyChart
|With MyChart, it's easy to view your child's medical records. No authorization form needed - you can read your child's medical record online just by logging into MyChart. You have full access to your child's medical record if your child is younger than 12. If your child is 12 to 17, you'll have partial or limited access, in accordance with Minnesota law, unless your child grants full access. If you would like to request a formal copy of your child's medical records, either for yourself or for a healthcare provider outside M Health Fairview, that's easy, too. Just visit the Sharing Hub page in MyChart to make the request.
Receiving Medical Records Another Way
If you prefer not to use MyChart, you can request to have your child’s records sent to the destination of your choice. To complete your request, you will need to fill out an Authorization for Release of Protected Health Information form.

Changing Your Child's Records
If you would like to request a change to your child's medical record:
- Complete a Request for Amendment to Health Information form
- Send the completed form to us at the address on the form
Transferring Medical Records
If your child's medical records are with M Health Fairview, they are available to all areas of M Health Fairview, and you don't need to transfer them. If you need to transfer records from another health system to M Health Fairview, or vice versa, ask your child's provider what steps you need to take to move your records. You can also contact our Medical Records team at releaseofinformation@fairview.org and fill out a Release of Information form to have your child's records sent from or sent to another health system.
Obtaining a Birth Certificate
If you need a copy of your child’s birth certificate or a change to a birth certificate, please contact the county health department where your child was born:
