How does NAVIGATE work?
Individuals can choose to enroll in one or more of the NAVIGATE services:
- Individual therapy
Therapy (counseling), aka Individual Resiliency Training, helps individuals learn about treatment options and new skills for coping with symptoms, all while focusing on resiliency and recovery. We assist in developing plans and goals, and act as a resource to help individuals attain them.
- Group workshops
Individual and family groups offer opportunities to meet others with similar experiences and learn new skills.
- Individualized medication evaluation and management
Medication helps to reduce symptoms and prevent relapse. Our prescribers use a shared decision-making approach when considering prescription medications.
- Supported employment and education
Professionals help individuals find, maintain or resume work and school activities. We might meet with individuals in the community on a regular basis (at their home, coffee shops, libraries, etc.), assess employment and educational goals, attend information sessions or tour schools, arrange and attend informational interviews, and obtain accommodations at school or on the job.
- Family therapy
Family therapy provides education on psychosis and its treatment to help family members support their loved one's recovery goals. We discuss topics including facts about psychosis and medication, coping with stress, relapse prevention planning, self-care and effective communication.
- Case management
We offer professional assistance in conjunction with community resources to address essential living needs like housing, insurance, and food.
Our program staff are highly skilled at providing both clinical and non-clinical support services, whether in our St. Louis Park clinic, the community, or in an individual’s home. M Health NAVIGATE is supported by funds provided from the Minnesota Department of Human Services.
